Surgical Technique
A surgeon performs a breast enlargement by inserting a breast implant either behind the breast tissue or partially or completely under the pectoralis chest muscle. The surgeon places incisions carefully in order to make the scars inconspicuous. Typical approaches for inserting the implants include around the areola (pigmented region around the nipple), under the breast or through the armpit. Surgeons do not enter through the belly button (umbilical approach) when inserting gel-filled implants. In fact, many breast implant companies void their warranties on implants for patients who receive their breast implants via the umbilicus. There are several different types of breast implants, each with its own benefits and disadvantages. All implants have a solid silicone shell. The shells may be smooth or textured like fine sandpaper. The content of the breast implants may be either saline (saltwater) or silicone gel. All modern implants in the United States are "cohesive" or like a gummy bear in consistency. The silicone in cohesive implants has a high cross linkage, which minimizes the chance of silicone oozing out of a ruptured implant. Implants may be either round or anatomic (teardrop) in shape. The method of implant selection and size, along with surgical approach for inserting and positioning breast implants depends on the preference and anatomy of the patient and the judgment of the surgeon. The shape and size of the breasts influences both the recommended after surgery treatment and the results. Most women have uneven breasts that are different in shape or size. Therefore, it is unlikely that both breasts will match each other perfectly after breast enlargement surgery. Breast implants along can address mildly sagging or droopy breasts (ptosis). Women with moderate to severe ptosis and decreased skin elasticity may want a breast lift in addition to implants.
